Reorganize docs: move specs and plans to doc/ subdirectories

Browse Source

Move doc/specs/ui.md to doc/spec/ui.md. Move plans/module-system.md to
doc/plans/. Add doc/spec/agents-runtime.md and docs/ reference specs.
